For those of you who have completed your online registration, kindly download the Health Declaration Form and fill it in.
If you have answered "Yes" to any of the questions, kindly obtain a medical certificate from a qualified doctor to prove that you are fit to participate in National Vertical Marathon 2010 and submit it to us on event day.
Do note that it is mandatory to submit this form and the necessary supporting documents to be able to take part in National Vertical Marathon 2010.
Regular Registration (CLOSED)
Steps for regular registration
- Download and print out the hardcopy form here OR grab a copy from our publicity booths.
- Fill up the registration form (Note that all mandatory fields denoted by + have to be filled).
- Fill up the Health Declaration Form. If you have answered "Yes" to any of the questions, kindly obtain a medical certificate from a qualified doctor to prove that you are fit to participate in National Vertical Marathon 2010 and submit it to us. (You may download and print out the Health Declaration Form seperately here.)
- Payments are to be made by cheque only and made payable to "NTU Student Union".
Kindly include the following information on the back of the cheque:
- Name
- NRIC Number
- Race category and
-"National Vertical Marathon 2010" |
- Please refer to the registration rates table below for the appropriate fees. Do note that cash cheques are not allowed.
- Enclose the following documents in a stamped envelope:
- Completed registration form
- Completed Health Declaration Form with Medical Certificate if necessary
- Photocopy of NRIC/Matriculation Card/Student Pass and
- Cheque |
